IMPORTANT NOTICE: This form must be read, understood and signed (accepted) by a parent or legal guardian before any child is allowed to participate in training or activities at Cross Court Tennis Academy (“the Academy”).
The Academy provides physical and sporting activities that require coordination, stamina, exertion, and may expose participants to inherent risks, including but not limited to sprains, collisions, dehydration or another injury.
By signing/accepting this waiver, I confirm that I, as the parent/legal guardian of the child named below, understand and agree to the following:
1. Voluntary Participation and Assumption of Risk
I acknowledge that participation in tennis training involves physical activity and inherent risks, both foreseeable and unforeseeable. These may include accidents resulting from falls, contact with other participants, or injury from equipment or surfaces. I voluntarily assume full responsibility for all risks associated with my child’s participation.
2. Medical Fitness and Disclosure
I confirm that my child is physically fit and has no known condition or illness that would prevent or limit safe participation. If any such condition exists now or develops later, I will immediately inform the Academy. I understand that the Academy does not provide medical evaluations prior to participation.
3. Supervision and Safety Rules
I understand that the Academy will provide qualified staff and safety protocols, and I agree that my child will follow all rules, briefings and coach instructions. I accept that non-compliance may result in removal from the activity for safety reasons, without refund or compensation.
4. Liability Waiver and Indemnity
To the fullest extent permitted by the applicable laws of the United Arab Emirates, I hereby waive, release and discharge Cross Court Tennis Academy, its manafers, staff, coaches, volunteers, and affiliates (“the Releasees”) from any and all liability for loss, damage, injury or expense that may arise during or as a result of my child’s participation, whether caused by negligence or otherwise.
This includes, but is not limited to liability arising from:
Use or misuse of equipment or courts
Accidental collision or injury during activities
Failure to follow safety rules and coach safety instructions and recommendations
Absence of medical care at the training facility
I further agree to indemnify and hold harmless the Releasees from any legal claims brought by third parties as a result of my child’s participation.
5. Legal Acknowledgement
I understand that this form is governed by the laws of the United Arab Emirates. If any portion of this waiver is found unenforceable, the remainder will still apply. I confirm that I am at least 18 years old and legally authorized to sign on behalf of the child.
Photography and Media Release
I grant permission for photographs or video footage of my child to be taken during sessions and used in Academy materials, including but not limited to social media, brochures and promotional materials. I understand this media may be used without additional notification or compensation. This consent can be withdrawn by written notice.
